How the program works:
- Complete the registration form below.
- When your friend or loved one calls to schedule an appointment to have their hearing tested, be sure they tell us that YOU referred them to guarantee your referral is acknowledged.
- If your friend or loved one purchases a hearing device, they must provide the referral card to the Hearing Instrument Specialist at the time of their purchase. Your friend or loved one will receive a $50 discount, and you will receive a $50 Visa Gift Card! (Discounts and gift cards `
- Your gift card will be mailed after your friend’s trial period has been completed, or you may pick it up in our office if you prefer.
- The more you refer, the more you receive! Gift Cards are limited to 1 per referral, but there is NO limit to the number of referrals you can send us!
You may also pick up a Refer-A-Friend card at any Hearing Healthcare Center office.

To qualify, you must have completed the above form, and the friend must make a qualifying purchase. All registrations must match the name and address provided to Hearing Healthcare Center upon their first appointment. Returned/invalid information will not be eligible.
